Mimostřevní salmonelové infekce

Mimostřevní ložisková infekce není častá, ale klinicky velmi významná manifestace salmonelové gastroenteritidy. Invazivní salmonelovou infekcí jsou ohroženi zejména pacienti s porušenou imunitou. Autor v přehledovém článku upozorňuje na nejčastější formy mimostřevní salmonelové infekce (primární bakteriémie, sekundární - enteritidou indukované bakteriémie, zažívací fokální infekce, nezažívací fokální infekce) a na dvou kazuistikách (salmonelová encefalopatie a salmonelová artritida) upozorňuje na možné diagnostické i terapeutické potíže. Autor, v souvislosti s větší vnímavostí k mimostřevním invazivním formám u pacientů s defektem imunity, upozorňuje na nový syndrom -Mendelovská náchylnost k mykobakteriální infekci- (MIM syndrom), který se u pacientů projevuje zvýšenou vnímavostí k infekcím, které jsou zapříčiněny BCG, netuberkulózními mykobakteriemi anebo kmeny Salmonella sp.
EXTRAINTESTINAL SALMONELLA INFECTIONS
Extraintestinal focal infection is not frequent, but clinically very important presentation of salmonellic gastroenteritis. Invasive salmonellic infection is jeopardy for patients with impaired immunity. Author-s overview warns from the most frequent extraintestinal salmonellic infections (primary bacteraemia, secondary enteritis-induced bacteraemia, digestive focal infection, non-digestive focal infection) and with two case reports shows possible diagnostic and therapeutic problems. With regards to greater susceptibility to extraintestinal invasive forms in patients with immunodeficiency author alerts to new syndrome -Mendel susceptibility to mycobacterial infection- (MIM syndrome), which presents in patients as increased susceptibility to BCG, non-tuberculosis mycobacteria or Salmonella sp. induced infections.
