Acute mastoiditis (AM) is a very rare but serious complication of acute otitis media (AOM) in young infants. Subperiosteal abscess is the most common complication of AM, and when occurring as the sole complication was successfully managed with antibiotics and surgical intervention. A four-week-old infant born of uncomplicated pregnancy was presented to the otolaryngology clinic with two days history of persistent agitated and irritability along with a protrusion of the auricle, redness and edema behind the ear. High-resolution CT scans of the temporal bone was performed; findings were normal except for the right coalescent mastoiditis, osteolytic lesions and postauricular subperiosteal abscess. Patient underwent right-sided antromastoidectomy and myringotomy with tympanostomy tube placement. Cultures of the purulent material grew Streptococcus pneumoniae sensitive to multiple antibiotic agents including the aminopenicillins.
