Streptococcus intermedius is a common commensal of human mucosa, but can also be a source of serious infections. We present the case of a 5-year-old girl with the development of a brain abscess who was hospitalized for several days of fever, vomiting and the development of meningeal irritation. Lumbar puncture showed mild pleocytosis in the cerebrospinal fluid with a predominance of neutrophils and proteinorachia. Culturally, immunologically and genomically, the cerebrospinal fluid was negative. The diagnosis of a brain abscess was made only after further deterioration of the overall condition by magnetic resonance imaging. PCR examination of the purulent contents of the lesion confirmed Streptococcus intermedius as the cause of the infection.
