Because of the routine vaccination pertussis became a second-line infection. However Bordetella pertussis was not eliminated from the environment. It causes atypical mild infection in teen agers and adults who are the source for susceptible infants. An active surveillance is necessary, correct diagnosis of these infections enables timely targeted antibiotic treatment. It is necessary to consider application of the booster dose of pertussis vaccine in adolescent to decrease the risk of exposed population.

Otogenic complications develop in the course of acute or chronic otitis media, they are health and life threatening and require precise diagnostic and adequate therapy. Strattera is a new drug approved for treatment of ADHD in Czech Republic in April 2007. Results of the comparison of atomoxetine (Strattera) and methylphenidate (Ritalin) treatment in children with ADHD provide initial evidence that atomoxetine produces comparable benefits to methylphenidate in reducing core ADHD symptoms in children. The rapidly increasing prevalence of obesity among children is one of the most challenging dilemmas facing pediatricians today. The prevalence of extreme obesity is increasing also in population of Czech children. The curability of obesity in childhood is very poor, approximately 10–30 %, so prevention of overweight in children should be the first line of management. Vaccination belongs to the most effective interventions in preventive health care. Precise knowledge of vaccine contraindications is necessary for their optimal use and avoiding false contraindications makes protection for the most vulnerable individuals possible. The only general contraindication (for the same vaccine) is a serious allergic (anaphylactic) reaction to a previous dose or to any component of the vaccine. Live vaccines are contraindicated in individuals with severe immune deficits. Pertussis vaccine is contraindicated with severe progressive neurologic disorders until clarification and stabilization of the condition.

Atopic epicutaneous tests (APT) are a diagnostic method that has been introduced into examination armoury of patients with constitutional dermatitis in recent decenium. The method is based on principal that inhalation or food allergens are able to trigger during epicutaneous aplication positive reaction of eczema type in spite of the fact that the allergen is high molecular and protein like. The method has been standardized. Indication to APT, the choice of individual allergens and mainly their correlation with prick tests and the level of IgE antibodies has been always a matter of discussion.
