Disorders of thyroid gland belong to the most frequent endocrinopathies in children and adolescents. The article gives a brief review on current knowledge of the pathogenesis, clinical manifestation, diagnostics and treatment of the most common non-oncological thyreopaties in pediatric clinical practice. In the group of congenital thyreopaties, the attention is paid especially to congenital hypothyroidism, nevertheless rare but serious congenital hyperthyroidism is discussed as well. The part of the article dealing with acquired thyreopaties is focussed mostly on autoimmune thyroid diseases. Differential diagnosis of goiter is given and iodine...

1st part: Diagnostic approach Early diagnosis of immunodeficiency makes possible early definitive therapy and avoids the complications of pretreatment infections. B-cell defects constitute the majority of primary immunodeficiencies. All are characterized by the reduction in or absence of immunoglobulins and/or specific antimicrobial antibodies. Consequently, substitution of immunoglobulin G (IgG) is the pillar of treatment. T-cell immunodeficiency defects become apparent as combined T- and B-cell deficiencies.

We present original and innovative Guidelines for prevention of overweight and obesity in infancy as a follow-up to the Guidelines for prevention and treatment of childhood obesity. The preventive guidelines are designed for use in everyday clinical practice by paediatricians for individual infants at risk of developing overweight, including their family background. They apply the basic principles of primary prevention of metabolic lifestyle diseases.

Early detection of visual defects in children is very important. However, it is the beginning of a demanding period for parents who have a child in therapy must ensure wearing glasses and/or contact lenses, often occluders. To manage this kind of therapy in young children up to 3 years, respectively up tu 6 years of age or in children with multiple disabilities is very complicated. We provide ideas how to cope with wearing glasses, contact lenses or occluder in children using our practical experience.

Barth syndrome (BTHS) is a gonosomal recessive disease characterized by heart involvement, neutropenia, myopathy, and growth retardation. The case report presents a nearly four-year-old boy who was hospitalized at our centre at the age of four months with a severe form of dilated cardiomyopathy. Treatment for heart failure was commenced. The investigations performed failed to show an infectious or other aetiology of the condition. As part of a broader differential diagnostic approach, the child underwent screening for metabolic disorders that showed 3-methylglutaconic aciduria. Barth syndrome was suspected and subsequently confirmed by molecular...

Authors presented case report of 6 years old boy with congenital generalised hypotonic syndrome accompanied by feeding problems and delayed motor milestones. Except the hypotonic syndrome boy expressed distal laxity, keratosis pilaris, and later mild contractures of knee flexors. Another sign of the disease were congenital hip dislocation. Mental development was normal. Blood level of kreatinkinase was mildly elevated (5.8 μkat/L). Due to the phenotype and the pattern in muscle MRI images of lower limbs genetic testing of COL6 gene was indicated and revealed causal in literature already described de-novo mutation in COL6A3 gene C6210 + 1G...

The authors present a case report of a 7-year-old boy with acute back pain with totally atypical clinical presentations. Acute pains were only in the night, non-steroidal analgesics were without effect. During the day the patient had no pain and was not limited his movement. Step-by-step were excluded injury, noninfectious systemic inflammation and metabolic causes. Graphical views confirmed the rapid progression of the expansive process caudal spinal cord. Infectious etiology was demonstrated subsequently, with a certain latency. Effective antimicrobial treatment resulted in total recovery.

The contribution mentions selected evaluating scales that can be used in the care of newborns and infants. They can be used to assess the state of the behavior of sick newborn, its possible manifestations of pain, agitation and sedation, and possible withdrawal symptoms manifested shortly after the birth or during hospitalization in a neonatal intensive care unit. Because they belong to the lesser known and rarely used, are described more detailed how to work with them.

Torsion of the testis the most common urological emergency and should be included in the differential diagnosis of scrotal trauma. Clinical suspicion of testicular torsion is a serious indication for prompt surgical exploration of post-traumatic acute scrotum. Ultrasound is the imaging modality of choice for the pediatric scrotum and provides excellent anatomical details and assessement of testicular perfusion by color Doppler without ionizing radiation. A case of unilateral post-traumatic testicular torsion, which resulted in operative detorsion and bilateral orchidopexy of viable testis, is presented.

and attachment to children Lactation counselling has long been well established in the Czech Republic, helping mothers to overcome breastfeeding difficulties. However, when providing advice, a lot of lactation consultants rely on personal experience rather than evidence-based medicine and the knowledge of fostering attachment between mother and child. In order to promote milk production (frequently referred to as "milk flow" by mothers and consultants), lactation consultants trained by the Mamila civil society organisation recommend the use of a drug with the active ingredient domperidone (Motilium in the Czech Republic; Costi in Slovakia)....
